Appointments
To schedule an appointment, please call (727) 581-8706 or e-mail a request and we will be happy to call you. We will make every effort to schedule an appointment time that is convenient for you.
Before your first visit, you will be given forms to complete. Your vital medical history along with other information will help our physicians and office staff serve you better. We will also attempt to secure copies of your previous ophthalmic medical records.
Office Hours
Our offices and telephone service are open from 8 am to 5 pm, Monday through Friday. One of our physicians is always on call for any after-hour emergency that may arise.
After-Hour Emergencies
If, after normal office hours, you find the need to speak with one of our physicians, please call our central telephone service, (727) 562-6839. After hours calls are forwarded to an answering service who will contact our physician on-call.
Should you have an emergency, please call 911.
Prescription Refills
Please contact your pharmacist for refills when your medicine is running low. It may be necessary for the pharmacist to check with your doctor for any needed authorizations. Please allow a minimum of 48 hours for the refill process.
Any refill requests received after office hours or on weekends will be handled the next business day. It is important to plan ahead and anticipate your medication needs especially when scheduling travel.
Financial Policy
Our policy is to collect fees at the time of service unless other arrangements have been made. For your convenience, we accept Visa, Mastercard, Discover, debit cards and American Express.
As participating providers of Medicare, we will file claims on your behalf. Under the Medicare program, each patient is responsible for a 20% co-payment after satisfying your Medicare deductible. As a courtesy, unless you request otherwise, we will also file any secondary insurance you may have.
Managed Care (HMO, PPO)
For patient convenience we participate in most major managed care (HMO, PPO) plans. If pre-authorization is required prior to your visit or treatment, please make sure this is completed with your plan provider or primary care physician prior to your appointment. Otherwise, you will be responsible for the entire cost of your visit or procedure.
Medicaid
As a participating provider with Florida Medicaid, eligibility and authorization must be documented in order for us to file your claims prior to any office visit or treatment.
